Sleep apnea is a sleep disorder where your breathing repeatedly stops and starts during sleep. The most common type, obstructive sleep apnea (OSA), happens when the muscles in your throat relax and block your airway. This can happen dozens or even hundreds of times per night, preventing you from getting restful sleep and putting strain on your heart.
Common signs include loud snoring, waking up gasping or choking, excessive daytime sleepiness, morning headaches, difficulty concentrating, and irritability. Your partner may notice that you stop breathing during sleep. The only way to confirm a diagnosis is with a sleep study, which can now be done at home with a simple device.
Oral appliance therapy uses a custom-fitted device — similar to a sports mouthguard or retainer — that you wear during sleep. The appliance gently repositions your lower jaw forward, which keeps your airway open and prevents the throat collapse that causes sleep apnea. It's FDA-cleared, silent, portable, and covered by most medical insurance.
For mild to moderate obstructive sleep apnea, oral appliance therapy is considered equally effective as CPAP by the American Academy of Sleep Medicine. For severe sleep apnea, CPAP may provide slightly better airway control, but many severe patients who can't tolerate CPAP see excellent results with oral appliances. The key is that a treatment only works if you actually use it — and adherence rates for oral appliances (~90%) far exceed CPAP (50–60%).

Most sleep physicians support oral appliance therapy, especially when CPAP adherence has been poor. The American Academy of Sleep Medicine recognizes OAT as a first-line treatment for mild-to-moderate sleep apnea and as an alternative for severe cases when CPAP fails.
