Our Coordinated Care Model

We treat oral appliance therapy as a collaborative process between the referring physician and the dental sleep medicine specialist. Your patient remains your patient — we provide the dental sleep medicine expertise and handle the appliance fabrication, titration, and follow-up.

The Referral Process

Referring a patient takes less than 2 minutes. We handle insurance verification, pre-authorization, and all administrative paperwork.

Send the Referral

Fax or submit an online referral with the patient's sleep study results, diagnosis, and a prescription for an oral appliance. Our e-fax number is (858) 898-5093. If the patient does not yet have a sleep study, we can arrange a home sleep test.

We Verify Insurance, Schedule & Treat

Within 24–48 hours, we verify the patient's insurance benefits, obtain pre-authorization if required, and contact the patient to inform them of coverage details and schedule the consultation. After evaluation, we fabricate a custom oral appliance, titrate it over 4–8 weeks, and confirm efficacy with a follow-up home sleep test. You receive a complete outcome report including pre- and post-treatment AHI data.

OSA Severity Scale

AHI of 0 to 5/hr. — within normal limits
AHI of 5.1/hr. to 14.9/hr. = mild OSA
AHI of 15.0/hr. to 29.9/hr. = moderate OSA
AHI of 30.0/hr. to 59.9/hr. = severe OSA
AHI of 60.0/hr. and above = markedly severe OSA

A custom oral airway device is an excellent OSA first-line treatment for patients with an AHI between 5.1/hr. to 40.0/hr.

Insurance note: CPAP failure, intolerance, or refusal documentation strengthens approval for patients with OSA.
